Literacy

When I started working with the non-profit world the first thing that jumped out at me was how many of the problems could be traced to unemployment.  Addiction, Haitians, returning troops from Afghanistan, abused women and children, Native American homeless kids the underlying issue would keep returning to unemployment.

+Annette Bosworth and I ( Chad Haber ) tried many things to create jobs for Haitians, returning military personnel, Native Americans this had the desired effect.  The job helped erase lots of the back ground noise or static as I  will refer to it from now on.  Once the static was removed the next common problem that was revealed was a below average reading level.

Incarcerated juvenile defenders eighty percent read two reading grades below grade level or a functionally iliterate.
California and Arizona are two of the several states that use early literacy test scores to predict required prison growth.

The data on reading and how to fix it is readily available.  The fMRI made it so that if you treat reading as any other medical condition the problem can be diagnosed and with aide of a doctor working with the team of education specialist a plan can be put in place to treat the underlying disease state.  The fMRI made it so the brain could be watched as the patient read a simple paragraph.  The fMRI results were matched with reading problems.  The good thing was that once a certain reading problem was identified the fMRI didn't have to be given to the next patient that presented with the same symptoms, we just simply didn't have the resources to pay for fMRI for every patient.  The lack of resources in this case was a benefit we had to look for cheap ways to get the results we wanted.

The goal was to get reading scores to grade level as quickly and as cheaply as possible.  

Preventive Health Strategies used a simple six question reading test developed by SAMPSA to flag individuals that had issues with reading.  The next step was to have a specialist do a screening with them to see if they fell into the broad diagnosis of dyslexia.  The Dyslexia diagnosis is enough to trigger most insurance companies to pay for medical treatment.  To date we have had no insurance companies actually pay for any test, diagnosis or treatment of reading disabilities.  

Preventive Health Strategies has to date screened over 1750 South Dakotans for reading deficiencies.  Preventive Health Strategies  found that a large percentage of females could be brought to grade level in reading with simple treatments for malnourishment.  The malnourished female population is well studied nationally no new research was required.  The treatment was cheap and easy to give under the supervision of a doctor.  The treatment had an immediate effect on all cognitive functions including reading, math and improved self self-worth (diagnosis of depression).   

Preventive Health Strategies at the same time took patients that did not have malnourishment issues and using the current research applied what fMRI has have taught PHS about reading disabilities.  The Primary care doctor would develop a plan with a reading specialist and the patient to address the plan.  Preventive Health Strategies found that the for under $10000 and six months total per patient they could be brought up to grade level provided they had an IQ in the normal range.

The results were amazing!  Real household income improved immediately (get and maintain jobs), compliance with other prescribed medications improved (resulting in fewer ER visits) in almost every area the individual reported a better life immediately not in five or ten years.  

 Pulitzer prize winning author Jared Diamond  states that a productive member of society will contribute five million dollars to the economy in their life time while a non-productive institutionalized member will cost the same society around seven million dollars resulting in a twelve-million dollar difference per individual.


Preventive Health Strategies did not understand the risk we put our primary care providers in when we asked them to start screening for literacy.  The power of the bureaucrats was underestimated.  The bureaucrats have the power and ability to gain the system with no real accountability or downside while the citizens of South Dakota pay the price.  

The complex systems (school, prison, health care, legal, mental health, insurance company) are full of interdependencies (hard to detect) in South Dakota.  This interdependence makes it hard to detect the underlying problem, simple problems are misplaced resulting in runaway chain reaction of events.

Preventive Health Strategies has naively acted with the assumption that organizations with mission statement of saving the state money and protect the lives of South Dakotans actually had these goals.  This naivety has resulted in more than one of Preventive Health Strategies primary care providers being investigated by the State of South Dakota.   The State of South Dakota has been ranked as the second most corrupt place in America by Fox News.

I Am

Trying to teach my kids that they need to take responsibility not just for their actions but for things they can improve.

Whenever you face a choice you can choose to take a positive action or no action.  Simple positive actions repeated over time determine the life you lead.


If they see a piece of garbage on the ground they need to stop and pick it up.  "I'm not the one that dropped it," is not acceptable.

This very simple lesson is important to me.  One person can make a difference.

When asked who is in charge "I am" should be the first and only answer that ever enters their minds.

A little effort each day will result in huge rewards over time.

We use piano to demonstrate this to them.  When they get a new recital song, the song is always very difficult.  By practicing every day little by little the song becomes possible.  Everyday  . . . no matter how inconvenient.

Is there something you know you should be doing?  What's stoping you from taking positive action?

I want my kids to know in their souls if they don't do it "who will?"

When you do positive things it increases the chances that positive things will happen to you.  Your smallest actions affect you and the people around you even if you are not aware of it.

The foundational prayer that we pray with our kids is "God, Help them do the next right thing!"  This takes a lot of courage.  When you are a teenager and your friends are experimenting with drugs and alcohol "No." Takes courage.
When you take responsibility for your employees actions this too takes courage.

When asked who is in charge around here? "I am!" Takes courage!


This week I read James 4.  The great thing about the bible is that every time I read it something new speaks to me.  The verse  "If anyone, then, knows the good they ought to do and doesn’t do it, it is sin for them."  

The important thing that jumps out in this verse is "for them!"  I truly believe that sin is part of the personal relationship that we each have with God. What is a sin for me might not be for you.  Evil is 

By seeing something that can be done better and not attempting to do something about it you have sinned.  


Walking by a piece of garbage on the street "that you see" and not stopping to pick it up is a sin!  This is a very simplistic example but an easy lesson for kids.

Bad at Names


I am really bad at names.  Everybody says they are bad at names but I really am.  Yesterday I was told that a little girl died in an orphanage Annette and I have helped in the past.  I spent a considerable effort trying to get straight which orphanage was being described.

Finally I said the one where my son played and laughed with the kids in the rain and I slept on the floor?  Instantly my sons face and this other wet face popped into my mind.

The moment is crystal clear in my memory of these wet happy faces gathering buckets of rain water.  The laughter and the joy that filled the air that night will live with me forever.

This morning I asked for a picture of the little girl that had died of typhoid.  The picture shocked me to my core.  The face is the same face that I envisioned.  This one really hurt.

I'm sorry I didn't ask your name then.  I'm sorry that even if I had I would not have remembered your name today.  I will remember your laughter and the joy you brought to my ten year old son forever.

Protecting the unborn




I'm looking for some feedback for a postcards. Using this picture. 

Last year Preventive Health Strategies helped single moms in every county in SD. PHS uses hand held ultra sounds. The impact this has on the mom is immediate. Once a mom sees her baby sucking on its thumb her life is for ever changed. PHS thanks you for the 453 uninsured single moms from South Dakota that you helped in 2012.



Description: Phs has given 453 mothers a first impression they will never forget. It is thanks to Phs and dr. Bosworth that these babies are alive and giving their mothers a new impression every day.

We just got done having a long talk with our boys. Walker Haber I don't like it when people feel "sorry" for me. Me "Why would they feel sorry for you?' W "Because we are homeless." Me "Do you think we are sad?" W "Sometimes?" Me "How do you think we move through that? We focus on the one beautiful thing that is or has happened in the day. Then we go forward being honest, open and willing." W "Isn't that why we are homeless?" I punted fed them CAKE! F...! Parenting is hard!

Listen God

My prayer this morning,

"Listen God. You and I both know that we ( +Annette Bosworth, MD ) are doing our best. 

You can see we are putting everything we have into this.

If you want us to do better you got to show up and do your part of the deal. 

The "Hell with it" you know we are not capable of doing anything less than our best because that's our job. 

Just give us a high five on our way through the pearly gates and say, "The two of you did an awesome job!"

I think that if we are allowed just a little bit of time to heal we will grow from all that has happened to us Post Traumatic Growth.  The issue is that we are not given that time everyday the attacks come from the same man.  This is called "economic terrorism" - attacking constantly so that there is no way to defend - historicaly the preferred weapon of corrupt governments.

I have not heard this term since I went through my confirmation class. I was confirmed United Methodist around 1982.

Prevenient Grace, "There is nothing this man can do to save himself. Outside help is his only hope." John Wesley founder of the Methodist Church.

I need to reread John Wesley's "Appeals to man of reason and religion." I think I would take more from it than I did when I was twelve.

Working with local community leaders in Haiti to make sure water is restored to 70,000 people Meaningful missions believes in direct support. Meaningful Missions finds the individuals in a community then makes sure they have what they need.

This community needed water. Meaningful missions talked to community leaders to find out what their water solutions were. The plans were well thought out and obvious. This was an obvious preventive health strategy, meaningful missions had the ability to help and we did.

The Preventive Health Strategies used only Haitians to do this project. The only support given was direct financial support and accountability. Training Meaningful missions local leaders to always follow up and make sure the work is completed and that nothing else is needed.

Eighteen local leaders showed up to "meaningful medicine" accountable.

The mayor explained to me "help tomorrow just means you are not going to help my people today!"

Bedtime stories from 2000 miles away

I have seen this feature advertised for months.  I honestly had no desire to try it.  I took my three young boys to Haiti.  My wife had to stay home.  My boys are well traveled.  I didn't think much about the trip.  My six year old started complaining about missing mom.

I tried Google Hang Out, I'm in Haiti.  Odd we have internet for free but no cell coverage($3/min) equals no cell coverage.

I sent her a Facebook message to try Google Hangout.  The whole process took a few minutes to get it to work the first time.  This was a great experience, the next night the fun of playing with the funny hats and sounds was done and Annette got to make her normal funny sounds.  The boys loved it (including me).

Today when I look at the kids in Haiti. I have more hope for them than I do my own kids.
The statement that has stuck with me about Annette's ordeal is "shocking to one's sense of fairness."

The Haitian kids don't expect things to be fair. But, they do expect to be able to have a rational and reasonable conversation.

They don't expect to be treated fairly by the government. The expectation by Haitians is that if you are noticed by an official (doing above average) your lively hoods, business, kids and land will be taken by the government.

When I am able to take a step back and look at the situation from Haiti. The most likely direction for the Haitian kids is up. My kids the next leg will likely be even further down.

Drop off

The little kids in the back of the tap-tap are being dropped off at a Haitian orphanage.

The whole experience brings back memories of taking livestock to market in the back of a pickup with my grandpa when I was a kid. The only difference being that at least some paperwork was done on the livestock(a check if nothing else).

Cleaning up the Garabage

This garbage pile has been her since 2008. Every time we drive by it Annette gets pissed "Why? Hasn't anybody cleaned that up?"

God works in amazing ways. This week I met the mayor of the community with the garbage pile.

Thanks to the many supporters on this trip the mayor is personally overseeing the cleaning up of this garbage pile today. On a Sunday!

Let me make that clear "today!"

God put us in the right place at the right time!

I wonder how many times the previous mayor was given the resources to do this job.

Happy birthday Annette!  It is because of you that this garbage is being cleaned up today.

Getting this garbage cleaned up is going to prevent lots of health problems both long term and today.

Michael St-Vilien

I'm sitting next to Michael in the back of a Haitian Tap-Tap (painted pickup/Haitian primary form of transportation) as I write this.

Michael Does an excellent job of negotiating for the organizations we work with in Haiti.

We have used Michael as a negotiator in government negotiations.

We have used Michael to calm an angry mob.

We have used Michael to calm angry clergy.

We have used Michael in negotiating supplies from Haitians and other organizations that have what we are lacking. We have had times that the thing we need is not available for sale in Haiti.

Michael is very well connected and was able to help us get what was needed. Michael has done this type of negotiating for us on things like fans, medicines, medical care. For those who have not operated in a third world country. I can not express the importance of this skill set. We have used other interpreters. The difference is like driving a 1982 Chevy Pinto and a brand new Ferrari they both do the same thing. But? How do you describe the difference to somebody that has never driven.

Michael understands the nuances if the English language this has come in most handy when we have been in danger and did not know it.

Personally the scariest place i have ever been in Haiti. It was after dark and the place that I had arranged to stay with my kids changed their mind.

Michael was not with us. Michael dropped everything and helped us to find a safe place to stay on a moments notice.

Michael is well connected in the Haitian community. Michael has helped us scale up quickly for events. Michael has with little notice get us 25 good English interpreters for a medical team.

I Believe!

I believe everything can be done better.

I believe that everything we do should be challenging the Status quo. How can we do this better? Why has it always be done that way? Why does it matter that that is the way it has always been done? What is the purpose? What is your motivation for doing that?  Are you getting the desired result?

I believe that every situation, individual, idea should be treated as the unique potential that they possess.
I believe that "U + I  > 2"

I have been told that I should never talk about:

1)Sex: as far as I can tell the reason for living
2)Religion: The purpose for living
3) Politics: Which I have just found out is more important than breathing.
